Terms and Conditions of Eliezer Mental Health Nursing Services

Welcome to Eliezer Mental Health Nursing Services (the "Clinic," "we," "our," or "us"). By using our telehealth psychiatric services, you (the "patient" or "you") agree to abide by the following Terms and Conditions (the "Terms"). These Terms govern your use of our telehealth services, and by accessing or using our services, you confirm your agreement to these Terms. If you do not agree with these Terms, you should not use our services.

Telehealth Services Overview:

Our Clinic provides remote telehealth psychiatric services, including consultations, diagnosis, therapy, and medication management, conducted through secure online video consultations, audio calls, or messaging. While our services are designed to provide convenient and accessible care, it is important to note that telehealth services have limitations. We are unable to provide in-person assessments or emergency care, and certain conditions may require a physical visit to a healthcare provider.

Eligibility Criteria:

To access our services, you must:

Be at least 18 years old (or the legal age of consent in your jurisdiction),

Reside in a region where our clinic is licensed to provide telehealth services,

Have access to a device with video and audio capabilities, and

Maintain a reliable internet connection.
If you do not meet these criteria, you may not be able to utilize our services.

Appointment Scheduling and Cancellations:

Appointments with our clinic are scheduled through our online booking system or by contacting our office directly. You are responsible for ensuring that you are aware of the correct appointment time, including taking into account time zone differences. If you need to cancel or reschedule an appointment, we require at least 24 hours' notice to avoid incurring a cancellation fee. Failure to attend an appointment or cancel without sufficient notice may result in a fee. Sessions typically last 30-60 minutes, but the length may vary depending on the nature of the consultation.

Fees and Payments:

The clinic charges for services rendered, and you are responsible for ensuring that payment is made. We accept insurance, cash pay, and credit/debit card transactions. Patients should confirm with their insurance regarding coverage for telehealth services. If insurance is applicable, it is your responsibility to verify coverage. Refunds for missed or canceled appointments will be processed according to our cancellation policy. For billing issues, please contact our office within 1-3 business days.

Messaging Frequency and Communication:

You may communicate with our clinic through secure messaging as part of your treatment plan. Messaging is typically used for follow-up questions, clarification on treatment plans, or administrative matters. Message frequency is generally limited to once or twice per week, but may vary based on individual treatment needs. You will not receive excessive or unsolicited messages.

Opt-In to SMS Text Messaging:

By opting in, you consent to receiving appointment reminders, treatment follow-ups, and other communications via SMS. Reply STOP to opt out at any time. Reply HELP for Customer Care Contact Information. Messages and data rates may apply. Message frequency will vary based on the nature of your engagement with our services.

Opt-Out of SMS Text Messaging:

If you no longer wish to receive SMS messages, you can opt-out at any time by replying STOP to any received message. You can also contact us at info@eliezerhealth.org to request that SMS communications be discontinued. Your opt-out request will be processed immediately.

Privacy and Confidentiality:

We take privacy and confidentiality seriously. All information shared during telehealth sessions is confidential and protected by relevant privacy laws. While we employ encryption and other security measures to protect your data, please be aware that no system can guarantee 100% security, and we cannot be held liable for any breaches that may occur due to technological issues.

Emergency Situations:

In the event of an emergency or urgent issue that arises during a telehealth session, we may refer you to appropriate in-person resources, but telehealth services are not appropriate for immediate crisis situations. You are advised to contact emergency services or go to the nearest emergency facility in the event of an emergency.

Termination of Services:
​
Both you and the clinic have the right to terminate services at any time. If you wish to discontinue services, you must notify us. Any outstanding balances must be settled upon termination. We may terminate services if there is non-compliance, disruptive behavior, or failure to make payments.

Intellectual Property:
​
All materials shared with you during your sessions, such as treatment plans, educational content, or resources, remain the property of Eliezer Mental Health Nursing Services. You may not record, reproduce, or distribute any content without written permission from the clinic.

Changes to Terms and Conditions:
​
Eliezer Mental Health Nursing Services reserves the right to modify these Terms and Conditions at any time. If significant changes are made, we will notify you via email or through our website. Continued use of our services after changes are made constitutes your acceptance of the updated Terms.

Governing Law:

These Terms are governed by the laws of the state of California. Any disputes arising from these Terms will be resolved through arbitration or mediation, rather than through the courts, in accordance with the rules of Eliezer Mental Health Nursing Services.
